Everything except the sink! In my backpack. She doesn't carry one. Frown
Seriously, it depends on the cache. I have actually got out of the car and found a cache with no GPS or anything. But usually we have.
Backpack
Home made walking stick (secret weapon)
H20
GPS
Extra batteries
small flashlight
camera
compass
whistle/thermometer
trade items
striking paper(TP)
Pen/pencil
rope
knife
lighter
lightweight rain suit
snacks
usually have extra clothes in the vehicle
I may be forgeting somethin'.
What's that saying? Shotgun, rifle, 4 wheel drive, and all this junk and a hillbilly geocacher can survive

Since the need for a paper Topo map depends on where I am headed, I don't always have one. But when Johnnie and I headed for Montgomery's Corner. We almost wore out the paper Topo map checking it against what we saw and what our GPS showed. When you are going this far off "trail", a paper topo and a compass are very helpful even with a GPS.

Also I tend to have a full change of clothes in the truck just in case, I need them when I get back.

Here is my list:

I don't go on multiple geocache hunts. I may do two or three in day way ever now and then, but most of the time I just hunt one on any given day. I plan my gear to the type hunt I am preparing for.

Drive-by
Less than 500 feet from parking.
I take nothing but a GPS

City and County type parks.
Less than a mile walk.
I will wear walking shoes and carry the GPS, hiking stick, trade goods, and a bottle of water.

Longer Hikes and mountain caching.
I plan each trip as to the estimated length and time of the hike.
Most hikes will include:
Camel Bak Pack with water
snacks
GPS
Cell Phone
Hiking Stick
Batteries
camera
compass
first aid

Longer and extreme Hike/geocaching
All the above plus
Maps
More first aid supplies
food- beans, ect.
knife
lighter
rain gear
tp
and a hiking partner

Like others have said
I have a change of rags and dry socks in the truck.
There is also a lot more outdoor gear in the truck, that I can take if the area looks like it will be needed.

Everytime I go out, I'm typically going further and steeper, and I also come back remembering something I need to add.

I agree that it depends on the cache as to what to carry. If I'm barely out of site of the car, GPS is all I need and maybe some swag if it's not a micro. For some of these deeper parks, add to that list a bottle of water.

My list so far for mountains and anything over 2 miles in:

Camelbak
First aid kit (with snake bite)
Gloves
Hiking stick (i opted for old school, basic hickory stick with a strap)
Swag
Poncho
4 pack of batteries
Foldup Pocket Toolkit (redneck and geek answer to a swiss army knife)
Mini-LED maglight (small but VERY bright and uses same batteries as GPS)
TP
BRE
Camera
Cellphone
Black Berry

Currently on my list to add:
Lighter (i'm an old boyscout, i can create fire, but i fear i'd be cussing like Tom Hanks in Castaway if forced to do so in an emergency now)
Mole skin
Compass (GPS has one, but if it were to break or get lost, an old school one could be handy)

Back at the truck, I'll have change of clothes, shoes, socks, more extensive first aid, maps, etc.

Yay! I finally have time to actually read forums! I'm late to this party, but my list is pretty much identical to Johnnie's, with a few minor exceptions. So, I'm cut-and-pasting-and-modifying your list, Johnnie. Hope you don't mind..

I always have my pocket knife and a pen on me, so those two go without saying.

Drive-by (Less than 500 feet from parking.)
- GPS, but sometimes not even that.

City and County type parks (less than a mile walk)
- GPS, PDA, sometimes a hiking stick.
- Occasionally a trade item, but not often.

Longer Hikes and mountain caching.
I plan each trip as to the estimated length and time of the hike.
Most hikes will include:
CamelBak with water
snacks
GPS
PDA
Cell Phone
Hiking Stick
Batteries
compass
first aid
smokes and lighter

Longer and extreme Hike/geocaching
All the above plus
Maps
Larger first aid kit
More snacks/MRE components
rain gear
tp
flashlight & headlamp & extra batteries for same
and a hiking partner
